Terms and conditions for Body Boost® Summer 2024 Pass
FIT4MOM North Atlanta Body Boost® Summer 2024 Class Pass
This contract is made between the client and FIT4MOM North Atlanta and is a pre-paid pass. Payment is required before attending class. This pass is non-refundable and non-transferrable. Any unused classes expire and will not roll over. The pre-paid pass is good for 16 Body Boost® classes, to be used between June 5, 2024 and August 24, 2024
AUTHORIZATION FOR
PAYMENT
I authorize FIT4MOM North Atlanta to initiate a one-time charge to the
account entered into Pike13 for payment.
AGREEMENT
By purchasing this pass, you are agreeing to the terms of this contract. Terms
of this agreement may change with proper advance written notice.
